Founded and operated by a dedicated Registered Nurse, Certified Case Manager, and Emergency Trauma Certified RN for over 28 years with extensive healthcare experience.

In the current role as Care/Case manager’s, the primary role as a provider is to coordinate and support patients, caregivers, and families in navigating the healthcare system, accessing necessary resources, and act as a central point of contact, patient advocate in developing and implementing care plans, monitoring progress, and providing education and support.
